What to expect the day of your appointment
The day of your appointment, you will need to bring your insurance card, a list of current medications, and a Diet Log. Your Diet Log will be sent to your insurance company along with a pre-determination packet, so it's important that you take your time preparing it. The diet log needs to be typed, and you will need to list every diet or exercise program you have tried in the past, along with a brief summary for each program stating how much weight you lost or gained.
